The timing of this study is remarkable.
April 9: FDA requires $LLY to provide post-marketing data on potential drug-induced liver injury (DILI) for Foundayo. Hy's Law plots. Cases >10x normal liver enzymes. NDA 220934.
April 15: Study in Cell Metabolism shows $NVO semaglutide has direct liver-protective effects. Reduces liver inflammation. GLP-1 receptors found on liver cells.
Two GLP-1 pills. Same price. Same receptor target.
One is under FDA investigation for potentially DAMAGING the liver.
The other is showing evidence of PROTECTING it.
This isn't coincidence. It's architecture.
Foundayo passes through the liver via CYP3A4. Every dose metabolized hepatically. Liver enzymes 3x elevated in trials. FDA now demands DILI tracking.
Wegovy pill bypasses the liver via SNAC. Semaglutide reaches the liver through systemic circulation AFTER absorption, at therapeutic levels, where it apparently activates protective GLP-1 receptors on hepatocytes. Not as a metabolic burden. As a therapeutic agent.
One drug treats the liver as a toll booth.
The other treats it as a patient.
And Wegovy already has FDA approval for MASH (fatty liver disease) since August 2025. This mouse study may explain the mechanism behind that approval.
Same class. Same receptor. Opposite liver effects.
Because the delivery path determines everything.
SNAC isn't just avoiding liver damage.
It may be enabling liver therapy.
