Ja kann ja sein, finde ich an der Stelle aber jetzt nicht bedenklich. Single Arm ist und bleibt eben nicht der neue Evidenzstandard, daher weist man explizit darauf hin. Wäre mal interessant zu sehen, ob diese Verklausulierung auch bei Keytruda in den Responses gegeben wurde…

Ja — das ist ein wichtiger Punkt, aber ich würde ihn eher als: „FDA markiert bewusst die wissenschaftliche Streitfrage“ und noch nicht als: „verstecktes negatives Signal“ interpretieren. Die relevante Passage in der PR ist sinngemäß: FDA habe bei der Filing-Entscheidung angemerkt, dass weiterhin Fragen bezüglich single-arm trials vs. randomized controlled trials bestehen. Das ist regulatorisch bedeutsam — aber man muss sehr genau lesen, wann und wie FDA das sagt. ⸻ 1) Das Entscheidende: FDA hat TROTZDEM akzeptiert Das ist der wichtigste Kontext. Die FDA hätte sagen können: * „Datensatz nicht ausreichend reviewbar“ → erneutes RTF. Oder: * „RCT zwingend vor Review“ → keine Acceptance. Hat sie aber nicht getan. Stattdessen: * Acceptance, * PDUFA-Date, * vollständige substantive review. Das heißt: Die FDA hält die Fragestellung offenbar für reviewwürdig, selbst wenn die Evidenzdiskussion offen bleibt. ⸻ 2) Warum FDA diese Sprache trotzdem verwendet Die Behörde schützt sich damit regulatorisch/juristisch. Deshalb schreibt FDA häufig: * offene Fragen bleiben bestehen, * Review wird diese adressieren, * Acceptance ist keine Vorentscheidung. ⸻ 3) Aber: Der Inhalt der Aussage ist real Das sollte man nicht kleinreden. FDA macht damit klar: Die Randomization-/Control-Frage ist NICHT erledigt. Und genau das bleibt wahrscheinlich die zentrale Hürde.
